Scheduling an appointment
When calling to schedule an appointment to meet with us in the office, please bring the following documents for your appointment:

  • Client’s dob and SS#
  • Physician’s name and address
  • Information about any home equipment
  • Physical & Mental status of client
  • Mobility
  • Diet
