Getting Started

We believe that every family we work with is unique and has specific needs.  In order to find the best match of services and staff for your situation, we take you through a short process to help determine how you would best be served by Stonehouse.

Initial information session
We always encourage you to come to our office to meet us and discuss your situation.  We know that having a face to face conversation allows everyone to feel comfortable with the person who is going to help you or your loved one.

If time doesn’t allow you to come to us, we are happy to discuss your family’s needs over the phone.

During this initial discussion we find out what your family needs and basic information about the person who needs care.  Based on our initial information session we can send a nurse to your home to perform a client assessment and a home assessment.  We will need doctors orders to start care.

Client Assessment and Home Assessment
The client assessment is performed by a registered nurse.  It is important that any caregiver with information about the client be present so that a full picture of the client’s medical, social and cognitive history can be attained.  The nurse will review the client’s medical history past and current and perform some basic testing.  With this information  and the client’s input, the nurse will determine a plan of care suitable for the client.  This will include a daily/weekly plan of services which will be performed by the appropriate service provider.

A home assessment will also be performed.  This assessment allows us to ensure that the home is safe, and determine a plan for emergency situations.  When a person endures a decline in health or a significant change in health status, there are new hazards where there once were none.  For instance, throw rugs can be a tripping hazard for a person who has just suffered a stroke or has trouble with balance.  We only make recommendations to make the home safer.

Beginning Care
Once the amount of care, level of care and service providers have been determined by the nurse and your family, care can begin.  Our care can range from a pal coming over to brighten and stimulate the day while tidying up the home and fixing lunch (companion) to round-the-clock nursing care.  We also provide physical, occupational and speech therapy if it is required.
